List of Student Concerns - Check List

The following is a list of concerns that students often have when they come  to the Counseling Center. If your are concerned about any of the following, or are experiencing problems not listed, do not hesitate to call. Please know that you are always welcome at the Counseling Center.  Call 938-4056 to arrange an appointment. Be sure to keep your completed checklist confidential, and share the information with your counselor or psychologist when you come for your first appointment.

Please mark each item with one of the following numerical responses, then print this out and take it with you to your appointment.

If you marked the number 3 at least once, counseling could benefit you.
 
If you marked the number 2 more than twice, counseling could benefit you.

If you marked the number 1 on any item, a one time consultation could help determine if you have a need for counseling
.

If you responded to items about depression, rape/assault, alcohol /drugs, out of control behaviors, or problems with eating or sleeping with a number 1 or higher, a consultation is indicated. PLEASE contact the Counseling Center today(938-4056).


If you are in crisis, come to the office and be seen by a counselor or psychologist the same day. Counseling Center office hours are Monday - Friday 8 a.m. - 4 p.m. After hours intervention services are facilitated by Public Safety.  Call 724-938-HELP.
